PGCert Long Term Conditions

  • Objectives Long-term conditions are those that cannot, at present, be cured, but can be controlled by medication and other therapies. This is an expanding area of healthcare as treatments and therapies are developed, increasing life expectancy for individuals with chronic illness or disease.

    The development of a National Service Framework for Long Term Conditions has identified specific areas of care and service delivery that require attention in order to allow individuals with long-term conditions to live as active and independent a life as possible.

    This innovative programme is designed to enable you to appraise the diverse range of political, health and social agendas linked to long term conditions, in order to:

    -evaluate the influences on chronic illness management;
    -utilise consultation skills to appraise and formulate the management of care for individuals who suffer from long term conditions logically;
    -facilitate the development of an advanced reflective practitioner so that you will be able to critically appraise knowledge;
    -apply and evaluate evidence in order to move practice forward and to contribute to the organisations’ objectives.

    The course is delivered on a modular basis, offering greater flexibility to course participants.

    Course Content

    You will undertake three core 20-credit modules:

    -Contemporary Issues in Long Term Conditions
    -Consultation Skills
    -Developing Professional Practice through Work-Based Learning

    Please note that all module details are subject to change.

    It is preferable for you to take these modules in the order listed above but, if you are unable to do so, the order can be negotiated.

    The programme will consider the political influences on the design and architecture of healthcare linked to long-term conditions. This will include the changing role of the practitioner as a leader and their influence on clinical policy.

    Clear links are made to caring for people with long-term conditions, creating an education framework that’s particularly useful for community matrons and case managers.

    Course Structure

    The Postgraduate Certificate in Long Term Conditions is delivered on a part-time basis over two years.
