PGCert Case Management of People with Long-term Conditions
ObjectivesThis is one of the first postgraduate programmes to be established in the West Midlands for case management for patients with long term conditions The course allows case managers to develop their skills in co-ordinating the health and social care needs for patients/clients with long term conditions The course is delivered in collaboration with the Strategic Health Authority and a neighbouring university (UCE Birmingham) - you will have the opportunity to work alongside, and share experiences and insight with, students from different health care backgrounds who are working in the field of case management Part of the programme will enable you to specifically develop your advanced health assessment skills in practice, and you will have the opportunity to work alongside experienced, practice-based facilitators The modules will be delivered by expert staff from the University and leading figures in the field of long-term conditions and chronic disease management You will have the opportunity to use interactive and computer-based learning materials as part of the course
Entry requirementsNormally, you should have a good Honours degree (or equivalent) in a health-related subject, as well as 3 years appropriate experience – substantial professional experience can substitute for graduate status Graduates from other disciplines, such as social science, may still be eligible for entry if they can show extensive experience in clinical practice You should have a good standard of written and spoken English (IELTS 6.0 or equivalent) You should also possess a professional qualification in health care, and be currently employed within the health sector - we will request a statement of support from your employer
